UCPath Notice: Verify Your Supplemental Health Plan Enrollment Data

To ensure a smooth transition of Aflac data into the UCPath system, the UCPath Center requested that employees verify the following personal information by September 30, 2022:

  • Name
  • Date of Birth
  • Social Security Number
  • Relationship Data

 

The following notice was sent to 2,609 UCLA Campus Employees and 2,043 UCLA Medical Center Employees. 


You are currently enrolled in one or more of Aflac’s supplemental accident, critical illness or hospital indemnity plans, and you elected coverage directly through Aflac’s website. The university recently converted enrollment records from Aflac to UCPath online. To prepare for this transition, the university took measures to ensure that your data was accurately transferred. Please log in to UCPath online to confirm that your name, date of birth, Social Security number and relationship data are correct for all your supplemental health plans in the UCPath online portal no later than September 30, 2022.

No further action is required if your supplemental health plan information was transferred correctly.

How to verify your supplemental health plan data:

  • Read the Review Enrollments in Benefits Summary Page job aid
  • Review your supplemental health plan data in UCPath online
  • Review your dependent’s supplemental health plan data in UCPath online
  • Verify that the dependent(s) name, date of birth, Social Security number and relationship data are correct
  • Submit an inquiry via “Ask UCPath” online only if the data is incorrect
  • ·Attach the completed Dependent Information Update form to your inquiry only if your dependent’s data is incorrect

How to Submit an Inquiry:

  • From the Benefits Summary Page, in the top right-hand corner, select the “Ask UCPath” button
  • Select “ Click Here to Submit an Inquiry”
  • On the inquiry page, select or type an answer in the following fields
    • Subject: type “Supplemental Health Plan Error”
    • Description: type a brief description of the enrollment data errors, including the plan type, errors, dependent information and necessary corrections.
    • Topic: select “Benefits”
    • Category: select “Supplemental AFLAC Plans”
    • Best contact phone number: enter your phone number
    • Best contact email: enter your email address
    • Attach the completed Dependent Information Update form, if applicable
    • Select “Submit”

UCPath will email you an inquiry number and expected resolution date upon submission. Additionally, you may check the status of your inquiry online at http://ucpath.universityofcalifornia.edu.

Please note:

  • UCPath will help correct data conversion errors due to the transfer, but you cannot make voluntary changes to plan elections at this time
  • New enrollments and requests for supplemental health plan coverage changes will now be made via UCPath online
