In an effort to better align with the UCLA Gender Recognition and Lived Name Policy, some CRU case submission forms were updated with new Employee Preferred Name fields.
CRU case forms that are submitted on behalf of other individuals (for example, HR/Admin forms) have new Employee Preferred First Name and Employee Preferred Last Name fields. These fields replaced the previous Employee First Name and Employee Last Name fields and are used to reference the employee in case-related communications.
In addition to preferred name, requesters are still asked to provide the employee's legal first and last name on a few forms:
- Glacier inquiries
- I-9 Verification Document Request Form
- Some HR/Admin forms
Note: The requester first and last name fields have not been impacted, this information will still be pulled from UCLA Single Sign On (SSO) when available.
