UCPath Notice: Request to Update Your Address

On November 3, the UCPath Center requested that 19 UCLA Campus employees and 1 UCLA Medical Center employees update personal information to satisfy a UC Health Benefit vendor requirement.


A review of your health benefits found that your address is not up to date. If your address is not corrected, access to care, payment of claims, and plan enrollment may be affected.

 

Please review your address for the following errors:

  • Incorrect or missing suffix for your street name
    • Abbreviated street name suffix (for example, Lane to Ln, Road to Rd, Avenue to Ave, etc.)
  • Apartment or unit number missing
  • “Apt” or “Unit” is not listed before the apartment/unit number
  • Incorrect spelling of your street name
  • Incorrect zip code
  • Not in the USPS Standardized format
  • Invalid home/mailing address
    • Employees and dependents enrolled in UC HMO plans must use California residential addresses (out-of-state, international, and campus addresses are not accepted)
    • PO Boxes are not accepted for residential addresses (they may be provided for mailing purposes only)
    • Postdoctoral Scholars enrolled in UCs PPO plans require a residential address in the United States (International and campus addresses are not accepted) 

To review and/or correct your address within UCPath, please follow this navigation path:

  • UCPath Home Screen > Employee Actions > Personal Information > Personal Information Summary > Home/Mailing Address > Change Home/Mailing Address

For dependents whose address is different than yours, follow this navigation path:

  • UCPath Home Screen > Health and Welfare> Dependent Coverage> Dependent 

select “Dependent” to review address details
