On May 2, the UCPath Center sent the following notice 595 UCLA employees regarding repayment of Aflac Supplemental Health Plan missing deductions.
- The communications indicate the employee’s total missed premium amount, catch-up deduction amounts to be withheld from future checks, and the corresponding paycheck dates.
- Employees received a customized message based on payment frequency as per the table below:

A recent review of your health benefits showed premium withholdings for your Supplemental Health Plans with Aflac were missed from your prior paychecks totaling {Recipient’s Total $ Missed Premiums}. The missed premiums (catch-up deductions) owed will be withheld from your upcoming paychecks to correct this error.
Your current pay period premium and the catch-up deduction for each paycheck listed below will be added together and show on your pay statement as one amount.
