UCPath Notice: Family Member Eligibility Verification

On May 5, we received the following alert from the UCPath Center regarding the Family Member Eligibility Verification process with UC health benefits vendor, UnifyHR.


Family Member Eligibility Verification

About 1,900 employees have not completed family member eligibility verification (FMEV) for their dependents with UnifyHR, which administers the verification process for UC. These dependents will be disenrolled from UC benefit plans on June 1, 2023.

UC requires all faculty, staff and retirees who enroll family members in their benefits coverage (medical, dental, vision, legal, dependent life, accidental death & dismemberment, etc.) to provide documents to UnifyHR to verify that dependents are eligible for these benefits. UnifyHR makes multiple attempts to contact the employees to obtain the documents via mail, email and telephone.

Employees who miss the verification deadline may still submit eligibility documentation to UnifyHR. Once UnifyHR verifies dependent eligibility, UCPath will be notified to reinstate the dependent in benefits as previously enrolled. Reinstatement is effective on the first day of the month following eligibility verification.

Note: FMEV reporting contacts at your local HR and/or benefits offices will receive a list of impacted employees on Monday, May 8, 2023. Please contact your local HR and benefits office for information about affected employees.
