Implemented the October 2021 update to the payroll configuration codes in Quicklinks

10/25/21

Implemented a new benefit eForm combining the following PDF into one online form:

  • Health Benefits Enrollment Form for Newly Eligible Employees (FR.008)
  • Health Benefits Enrollment Form for Life Events (FR.065)
  • HSA, Life Insurance, Voluntary Disability and AD&D Change Form (FR.066)
  • Late Enrollment Request (FR.006)

The new form can be used by employees eligible for staff/faculty benefit programs when self-service is not available.  Access the forms from UCPath > Forms Library > Access Forms > Benefits eForms: Submit New Form

10/25/21

Redesigned the off-cycle dashboard to display all off-cycle requests, not just those submitted via payroll request forms.  Additional improvements include:

  • Simplified status terminology
  • Improved filtering
  • A new “Request Type” column to indicate final pay, off-cycle or overpayment

The improved dashboard is available in QuickLinks. Reference the updated job aid in the Location Help Library for more details.
